Description: Applications for an Ontario Birth Certificate can be obtained at the City of Stratford, City Clerk’s office, 1 Wellington Street.
Forms can also be downloaded by clicking on the link below.

Completed applications can be mailed directly to the Office of the Registrar General. This method takes approximately 6 to 8 weeks.
Applications can also be ordered online at www.serviceontario.ca or at Service Ontario Offices. Please be sure to read the instructions thoroughly.

Please note that the City of Stratford does not issue Birth Certificates.

Package: Certificate of Insurance

Description: Purchasing
The contractor shall take out and maintain, during the life of the contract, such insurance applicable to the project and/or service they are performing. All insurance shall be carried with insurance companies authorized to do buisness in the Province of Ontario. The Corporation of the City of Stratford shall be added as additional insured. A certified copy from your broker shall be forwarded to Purchasing. Please complete the attached 'Certificate of Insurance - Purchasing'.

Social Services, Public Housing Division
When doing business with Social Services, Public Housing Division, the same information as above applies, however, we require the attached 'Certificate of Insurance - Public Housing' to be completed. This names City of Stratford and Perth and Stratford Housing Corporation as additional insured’s.

Clerks
A Certificate of Insurance is required by the City of Stratford for various purposes including but not limited to all events held on municipal property, streets or sidewalks, encroachments and rental of the City Hall Auditorium. The Certificate of Insurance must be completed by your insurance company, naming “The Corporation of the City of Stratford” as an additional insured with minimum amount of insurance coverage of $2 million dollars or as required by the City’s insurer, per occurrence. Package: Getting Married

Description: This package contains background information and forms to apply for a marriage licence from City Hall. The licence, once issued, is valid anywhere in the province for a period of 3 months.

To apply for a marriage licence, print off the blank marriage licence application form, complete all sections and sign it. Bring the completed application form, licence fee and the required documents to the Clerk's Office at City Hall, 1 Wellington Street, Stratford. Please refer to the Marriage Licence Requirements for the documents that you will need to show when applying for your marriage licence. To arrange a civil marriage ceremony at historic Stratford City Hall, please review the attached information on Civil Marriage Ceremony Requirements. To book a ceremony, contact the Clerk's Office at 519-271-0250 ext 237 during business hours.

To obtain a copy of your marriage certificate after you are married, complete the 'Application for a Marriage Certificate' and forward to the Province of Ontario. Applications can also be ordered online at www.serviceontario.ca or at Service Ontario Offices. Please be sure to read the instructions thoroughly.

Description: The City requests that organizations requiring financial assistance in 2011 should submit their grant applications by October 15, 2010.

Please contact the Office of the Director of Corporate Services at 519-271-0250 ext. 202, for further information on application requirements for a 2011 grant and the grant process.

Grant submissions will be reviewed at meetings of the Finance and Labour Relations Sub-Committee sometime in January 2011. These dates have not been determined, but will be published when available. Members of the public are welcome to attend and observe these meetings.

Package: Parade/Street Event Application

Description: All applications must be submitted at least 8 weeks prior to your event. This package contains an application form and blank certificate of insurance to apply to conduct a parade, street event, walk-a-thon, running event or other event on a city street or sidewalk. The Procedures outline the new process for submitting your application to the City of Stratford. Under this new process, all applications are submitted directly to the City Clerk's Office, Ground Floor of City Hall, rather than taking your application to each department for comments. Comments and approvals will now be obtained through an internal process.

To apply, print off the application form, complete all sections on the first and second pages and submit it to the Clerk's Office at City Hall along with the required map. The Certificate of Insurance must be completed and signed by your Insurance Company.

Description: The City of Stratford requires that all dogs and cats be registered with the City on an annual basis.

This package contains information about the fees and requirements for registering/licencing your dog(s) and cat(s) with the City of Stratford. The application form can be completed, printed off and mailed to the Clerk's Office, City Hall, P.O. Box 818, Stratford ON N5A 6W1. The City will mail a receipt and the tag to you. Cheques should be made payable to the 'City of Stratford'. Registrations can also be done in person at the Clerk's Office, 1 Wellington Street, Stratford where Interac and credit card services are now available.

Dog and cat registrations can also be renewed in person at vet clinics and pet stores in Stratford. Exotic pets must be registered with the local branch of the OSPCA. There is no fee to register your exotic pet. The OSPCA can be contacted at 519-273-6600.

Package: Property Tax Rebates for Vacant Commercial and Industrial Buildings

Description: Vacant Units – Bill 140 has also changed the taxation policies for vacant space within commercial and industrial properties. Commencing with 2001 all buildings are assessed and taxed as fully occupied. Property owners must apply to the municipality for vacant space rebates by February 28th of the year following the vacancy.

To apply for the rebate, complete the application below and return to City Hall, Tax Dept., or fax completed form to (519) 271-4357. For further details, please call the Tax Department at (519) 271-0250 extension 213.

Package: Retail Business Holidays Act and Holiday Shopping

Description: This package contains an application form and background information to apply for a municipal exemption to permit a retail business to open for business on certain holidays in Stratford's designated tourism season.

Small retail businesses in Stratford are able to open on Victoria Day, Canada Day, Labour Day and Thanksgiving Day under certain conditions set out in By-laws 84-90 and 149-92, provided they are directly associated with tourism or rely on tourists visiting Stratford for business on a holiday. Retail businesses over 2,400 square feet in size, must apply for a municipal exemption in order to open for business on holidays during Stratford's designated tourism season.

To apply, print off the application form, complete all sections and submit it to the Clerk's Office at City Hall along with the required information and application fee.

Package: Taxes - Pre-Authorized Payment Plan

Description: Choose the Plan that’s Right for You:
Option 1: Monthly

This plan runs from January to December with withdrawal on 1st day of each month. The first six monthly payments are based on the previous year’s tax levy. The final six monthly payments are adjusted to reflect the actual taxes for the year. You will be notified of any changes in your monthly withdrawal amount.

Option 2: Instalment Due Dates
Your bills will be sent to you as usual. The instalment amount will be withdrawn from your account on the instalment due date. To have the plan in place for the current tax instalment, please ensure this application is received in our office at least 15 days before the first due date.

For a joint account, all depositors must sign if more than one signature is required on cheques issued against the account.
